How Long Do Dogs and Cats With Diabetes Live? Life Expectancy and Prognosis
A diagnosis of diabetes mellitus does not automatically mean that a dog or cat has only a short time left.
Some diabetic pets live for years after diagnosis, while others develop serious complications soon after the disease is discovered. The difference is often less about the word “diabetes” itself and more about the animal’s age, general health, response to treatment, glucose control, and the presence of other diseases.
There is no reliable answer such as “a diabetic dog will live two years” or “a diabetic cat will live five years.” Survival studies show considerable variation between patients and between treatment settings. For example, a large UK primary-care study of diabetic dogs reported a median survival of 15.6 months from diagnosis, increasing to 20.2 months among dogs that survived at least seven days after diagnosis. Other studies have reported substantially longer survival in selected or well-managed dogs.
Cats can also live for several years after diabetes is diagnosed. In one recent study of diabetic cats, 82% of cats with follow-up until death survived for more than one year, 35% survived more than three years, and 13% survived more than five years. The median survival time was 773 days, although survival was longer among cats that achieved diabetic remission.
These numbers are useful for understanding the disease at a population level, but they cannot predict how long an individual pet will live.
How Long Do Dogs With Diabetes Live?
The prognosis for a diabetic dog is highly variable.
In a large UK study involving dogs diagnosed with diabetes in primary-care practice, the median survival time was 15.6 months from diagnosis. When dogs that died within the first seven days were excluded, the median survival increased to 20.2 months. The study also found that starting insulin treatment was strongly associated with improved survival.
This does not mean that a diabetic dog can be expected to live only 15–20 months.
Median survival is a population statistic. Half of the dogs in a particular study lived longer than the reported median and half lived less. The population also includes dogs diagnosed at different stages of disease, with different ages, complications, concurrent diseases, and levels of treatment.
Some dogs live considerably longer than two years when diabetes is successfully stabilized and other health problems are controlled.

A particularly important point from canine survival research is that the animal’s condition at diagnosis matters. In the UK primary-care study, dogs aged 10 years or older, dogs with very high blood glucose at diagnosis, and dogs with previous glucocorticoid exposure had higher hazards of death.
Can a Dog With Diabetes Live for Many Years?
A diagnosis of diabetes does not by itself define the remaining lifespan of a dog.
The most favorable situation is usually a dog whose diabetes can be stabilized, whose insulin requirements are reasonably predictable, and who does not have severe concurrent disease. Once the dog is eating reliably, maintaining body weight, drinking and urinating closer to normal, and avoiding major glucose fluctuations, long-term management becomes much more practical.
One of the most important changes after treatment is not necessarily that the blood glucose becomes “perfect.” The clinical goal is to maintain acceptable diabetic control while avoiding dangerous hypoglycemia and keeping the dog feeling well.
In practice, a dog that has stable weight, a good appetite, normal activity, controlled thirst and urination, and no recurrent ketoacidosis can have a substantially different outlook from a dog that repeatedly becomes dehydrated or ketotic.
Can Diabetes Shorten a Dog’s Life?
It can, particularly when the disease is poorly controlled or complicated by other serious conditions.
Diabetes itself is not always the immediate cause of death. Problems such as diabetic ketoacidosis, severe hypoglycemia, pancreatitis, infections, kidney disease, cancer, or endocrine disorders may become more important to the overall prognosis.
This distinction is important when discussing life expectancy with an owner.
A diabetic dog may eventually die from an unrelated age-related disease years after its diabetes has been diagnosed. In other cases, uncontrolled diabetes or its complications may be a major factor in deterioration.
The diagnosis therefore needs to be considered in the context of the entire patient rather than as an isolated disease.
How Long Do Cats With Diabetes Live?
The outlook for cats is somewhat different because diabetic remission is possible in a substantial proportion of feline patients.
Several studies have documented prolonged survival in diabetic cats, particularly when diabetes is diagnosed and managed effectively.
A recent study of 174 cats with newly diagnosed diabetes reported a median survival of 773 days among cats with follow-up until death. However, the distribution was broad: 82% survived more than one year, 35% more than three years, and 13% more than five years.
Another study of 185 diabetic cats treated with protamine zinc insulin reported a median survival time of 1,488 days, or approximately four years. Survival ranged from very short periods to more than ten years.
These studies illustrate why giving one fixed life-expectancy number for diabetic cats is misleading.
Different studies involve different populations, treatment protocols, ages, disease severity, follow-up periods, and definitions of remission.
Can a Cat With Diabetes Go Into Remission?
Yes. This is one of the major differences between feline and canine diabetes.
Some cats can enter diabetic remission after appropriate treatment, meaning they maintain acceptable blood glucose levels without ongoing insulin therapy for a period of time.
Remission does not necessarily mean that the diabetes has been permanently cured.
A cat that enters remission can relapse, particularly if the underlying metabolic problems persist or diabetes becomes poorly controlled again. In one recent study, 47% of newly diagnosed cats achieved remission, but 40% of those cats subsequently relapsed.
This is why a cat that no longer requires insulin still needs monitoring.
What increases the chance of remission in cats?
Several factors have been associated with remission, although no individual factor guarantees it.

Studies have found associations between lower-carbohydrate feeding strategies and improved remission or survival, although diet should always be selected according to the individual cat’s body condition and concurrent diseases rather than by carbohydrate content alone.
Does Remission Mean a Cat Will Live Longer?
Remission appears to be associated with longer survival in several feline studies.
In the recent study of 174 diabetic cats, median survival was longer in cats that achieved remission. At four years, median survival was 959 days in cats that achieved remission compared with 746 days in cats that did not. The researchers also found an association between remission and longer survival at later follow-up points.
However, this should not be interpreted as proof that remission alone determines lifespan.
Cats that achieve remission may differ from non-remission cats in other clinically important ways. Disease severity, age, concurrent disorders, treatment response, and the ability to maintain good metabolic control can all influence outcome.
Does Diabetes Kill Cats Quickly?
A newly diagnosed diabetic cat can remain relatively bright, eating well, and functioning normally despite markedly abnormal blood glucose.
However, some cats present in a much more serious metabolic state. Diabetic ketoacidosis, severe dehydration, anorexia, vomiting, or profound weakness changes the immediate prognosis and requires urgent treatment.
A recent UK primary-care study found that approximately 20% of cats in its diabetic cohort died within 30 days of diagnosis, with most of these deaths occurring early after diagnosis. This illustrates an important point: the first weeks after diagnosis can be particularly important, especially when treatment is not started or when severe concurrent disease is present.
At the same time, the same body of evidence shows that many cats survive well beyond the initial diagnosis and can live for several years.
Why the First Weeks After Diagnosis Matter
The initial stabilization period is often more informative for prognosis than the diagnosis itself.
A cat or dog that initially arrives dehydrated, ketotic, anorexic, or severely debilitated needs a very different approach from a patient that is still eating, hydrated, and clinically stable.
During the first weeks of treatment, the veterinarian is looking for several things:
- resolution of excessive thirst and urination;
- stabilization or recovery of body weight;
- appropriate appetite;
- improvement in activity and general condition;
- acceptable glucose control;
- absence of clinically significant hypoglycemia;
- absence of ketones or ketoacidosis;
- predictable response to insulin.
In dogs, stabilization within the first few months has been associated with a markedly better outcome. In one primary-care study, dogs that stabilized within three months had a median survival of about 20.5 months, compared with 2.5 months in dogs that failed to stabilize during that period.
That finding should not be used to predict an individual dog’s lifespan, but it demonstrates how important successful stabilization can be.
Does Blood Sugar Level Determine How Long a Diabetic Pet Will Live?
Owners often focus on a glucose value because it is the easiest number to see on a laboratory report or home glucose meter.
But prognosis is not determined by one glucose measurement.
A glucose value needs to be interpreted together with:
- clinical signs;
- body weight and body condition;
- insulin dose;
- glucose curves or other monitoring results;
- fructosamine when appropriate;
- urine glucose and ketones;
- appetite and hydration;
- concurrent diseases;
- episodes of hypoglycemia;
- previous diabetic ketoacidosis.
Very high blood glucose at diagnosis can be associated with a poorer prognosis in dogs. In the UK study, dogs with blood glucose above 40 mmol/L at diagnosis had a higher hazard of death than dogs with glucose below 20 mmol/L.
That does not mean that a dog with a glucose concentration above a particular number has a predetermined life expectancy.
It means that the initial metabolic state can provide useful prognostic information when interpreted as part of the whole clinical picture.
What Complications Can Shorten the Life of a Diabetic Dog or Cat?
The most important complications are usually those that interfere with metabolic stability or reflect significant underlying disease.
Diabetic ketoacidosis
DKA is a medical emergency characterized by severe metabolic disturbance, ketone production, dehydration, and often gastrointestinal or systemic illness.
A diabetic pet that develops vomiting, marked weakness, dehydration, anorexia, or abnormal breathing needs urgent veterinary assessment.
Hypoglycemia
Hypoglycemia can occur when the insulin dose is excessive relative to food intake or the animal’s current insulin requirement.
Signs may include weakness, unusual behavior, tremors, disorientation, seizures, or collapse.
Repeated or severe hypoglycemia is dangerous and is one reason insulin doses should not be changed casually based on a single glucose reading.
Pancreatitis
Pancreatic disease can be closely associated with diabetes, particularly in dogs. Pancreatitis may make diabetic regulation considerably more difficult and can become a major factor in the overall prognosis.
Chronic kidney disease
Kidney disease is especially relevant in older cats, which are also the population in which diabetes commonly occurs. The presence and severity of chronic kidney disease can influence both treatment choices and overall survival.
Infections
Poorly controlled diabetes can predispose animals to infections, including urinary tract infections and other bacterial infections.
Cataracts in dogs
Cataract formation is a common complication of canine diabetes. Cataracts can progress rapidly and may result in vision loss even when the diabetes itself is being treated.
Diabetic neuropathy in cats
Some diabetic cats develop peripheral neuropathy, often recognized as a plantigrade stance in the hind limbs. This complication can improve with effective diabetic control in some patients.
What Matters More Than the Diagnosis When Predicting Survival?
When an owner asks, “How long will my dog live with diabetes?” or “How long will my cat live with diabetes?”, the most useful clinical answer depends on several individual factors.

Can a Diabetic Pet Still Have a Good Quality of Life?
For many owners, this is actually a more useful question than life expectancy.
A well-controlled diabetic dog may continue to walk, play, eat normally, sleep comfortably, and interact with its family much as it did before diagnosis.
A diabetic cat may continue grooming, hunting toys, jumping, seeking attention, and maintaining a normal household routine.
Treatment does require commitment. Insulin injections, feeding routines, glucose monitoring, veterinary appointments, and adjustments to treatment become part of everyday life. But the presence of a treatment routine does not automatically mean poor quality of life.
In feline studies, better perceived quality of life has been associated with longer survival, while remission has also been associated with improved quality of life.
When Should a Diabetic Pet Be Rechecked?
Diabetes is not a condition that can be stabilized once and then ignored.
Insulin requirements can change with:
- changes in body weight;
- dietary changes;
- infection;
- pancreatitis;
- other endocrine diseases;
- changes in activity;
- medication changes;
- progression of concurrent disease;
- diabetic remission in cats.
A pet that suddenly starts drinking more again, loses weight, becomes unusually hungry, stops eating, vomits, or becomes weak should be reassessed rather than simply receiving a higher insulin dose at home.
In particular, increasing insulin without veterinary guidance can be dangerous. A pet may appear hyperglycemic because of a temporary problem while actually requiring less insulin than before.
What Is a Realistic Life Expectancy for a Diabetic Cat or Dog?
There is no single number that applies to every diabetic pet.
Based on published studies, however, it is reasonable to say:

The most important practical point is that a median is not a deadline.
A dog that has lived 18 months after diagnosis has not “reached the expected end” of its diabetic lifespan. Likewise, a cat that has been diabetic for five years is not unusual enough to invalidate the diagnosis or prognosis. Individual outcomes can differ substantially from population averages.
When Is Diabetes Considered End-Stage?
Diabetes itself is not usually described simply as “end-stage” in the same way as some progressive organ diseases.
A diabetic pet may have poor control but still respond well to a change in insulin management, diet, or treatment of an underlying disease.
The situation becomes much more concerning when diabetes is accompanied by severe, irreversible concurrent disease or repeated life-threatening complications.
Examples include:
- recurrent diabetic ketoacidosis despite appropriate management;
- severe chronic kidney disease;
- advanced cancer;
- severe pancreatitis with poor recovery;
- persistent anorexia;
- repeated severe hypoglycemia;
- progressive neurological or systemic disease;
- sustained deterioration in quality of life despite appropriate treatment.
At that point, prognosis should be discussed on the basis of the whole patient, not the diabetes diagnosis alone.
The Most Important Prognostic Signs at Home
Owners can often provide the veterinarian with useful information simply by watching the animal’s daily routine.
A diabetic pet generally has a more favorable day-to-day picture when it:
- maintains a stable body weight;
- eats reliably;
- drinks and urinates at a reasonably stable level;
- remains active and comfortable;
- does not have recurrent vomiting;
- has no persistent ketones;
- does not experience episodes of hypoglycemia;
- maintains normal interaction with the family.
Conversely, new weight loss, increasing thirst, reduced appetite, vomiting, weakness, dehydration, or a sudden change in behavior should prompt reassessment.
These changes are more informative than a single isolated glucose value.
The Bottom Line: How Long Can a Dog or Cat Live With Diabetes?
Dogs and cats can live for years after a diagnosis of diabetes, but there is no reliable fixed life expectancy.
In dogs, published survival studies commonly report median survival around one and a half to two years in primary-care populations, although individual dogs may live substantially longer. Successful stabilization and consistent insulin treatment are important prognostic factors.
In cats, survival can be several years, and remission is possible in a significant proportion of newly diagnosed patients. Studies have reported median survival times of approximately two to four years, depending on the population and treatment approach. Cats that achieve remission tend to have longer survival in observational studies.
The number that matters most for an individual patient is not the average from a study. It is whether the pet can achieve stable diabetic control, maintain body weight, avoid serious complications, and continue to have a good quality of life.
For that reason, when discussing prognosis with an owner, I would not start with a number of months or years. I would first look at the patient’s age, body condition, current clinical signs, insulin response, blood and urine results, concurrent diseases, and how well the diabetes can be stabilized. Those factors provide a much more meaningful estimate of what the future may look like for that particular dog or cat.
